Starter application below (if you’d like to print, fill out and bring with you to the conference)

Additional paperwork to bring to conference: child’s birth certificate, shot & health (physical) records, social security card, parent photo id, past two years of report cards if applicable

There will be additional forms to read and sign at the conference

Pay registration fee $150 and testing fee $50 (K-12th grade)

Administration will approve your application, and call you to make a date for testing.
